Congenital Lobar Emphysema

• Overexpansion of one or more lung segments
• Affected lobes LUL> RML> RUL
• Partial bronchial obstruction allows expansion but not exhalation
  • Intrinsic – dysplastic cartilage, web
  • Extrinsic – compression from large PA or other mass
• May appear as solid mass in early newborn period before fetal lung fluid is resorbed
• Surgical resection not always necessary
• Symptoms worse in 1st month then stabilize
• Use low ventilator settings if mechanically ventilated
• May observe if minimal symptoms
