Congenital Lobar Emphysema
- Overexpansion of one or more lung segments
- Affected lobes LUL> RML> RUL
- Partial bronchial obstruction allows expansion but not exhalation
- Intrinsic – dysplastic cartilage, web
- Extrinsic – compression from large PA or other mass
- May appear as solid mass in early newborn period before fetal lung fluid
is resorbed
- Surgical resection not always necessary
- Symptoms worse in 1st month then stabilize
- Use low ventilator settings if mechanically ventilated
- May observe if minimal symptoms
