CHF (partially complete...)

A previously healthy toddler presents w/ signs of CHF...

• a de novo presentation of congenital heart disease would be unusual, one exception being the anomalous origin of a coronary artery from the pulmonary artery which might present at any age w/ a cardiac infarct and CHF.  Pistol Pete Maravich died from this cause while playing basketball after his pro career ended.
• the most common cause is myocarditis; limited data to suggest that IVIG may be of some benefit and corticosteroids may be detrimental
  • coxsackie virus
  • adenovirus
• metabolic/genetic diseases may lead to a dilated or HCM that present w/ CHF
• high-output states like sepsis or severe anemia may cause CHF
